What does this screening tool measure?

The PHQ-9 is a depression screening questionnaire. It summarizes reported symptoms over the past two weeks, with scores from 0 to 27. A clinician can interpret the result alongside your history and daily life; the score alone is not a diagnosis.

Depression can look different for everyone

Depression can involve low mood or a loss of interest, along with changes in sleep, appetite, energy, or concentration. Not everyone experiences the same symptoms. How long symptoms last and how they affect your life matter, too.

Understanding the PHQ-9 score

The nine scored questions produce a total from 0 to 27. The usual symptom ranges are minimal (0–4), mild (5–9), moderate (10–14), moderately severe (15–19), and severe (20–27). The additional daily-life question does not change your score. A clinician interprets the result alongside your history and circumstances.

A low score does not make your concerns less real

This tool asks about specific experiences during a particular time period. It does not measure everything you may be going through. If symptoms persist, change, or interfere with daily life, an appointment can be useful even when the score is low. You can print your result to bring with you.

You have options for support

Start with a professional you can speak openly with, such as your primary care provider or a mental health clinician. Depression treatment may include psychotherapy, medication, or both. Your needs, preferences, health history, and access to care all help shape a treatment plan.
